As you are aware, Dr. Désirée Ratner and Désirée
Ratner, M.D., P.C. are out of network with your insurance company.
Dr. Ratner does not participate with any insurance plans and is considered
an out-of-network provider. As a result, payments for services are due at
the time services are rendered. It is important to note:

Your insurance is a contract between you, your employer, and the
insurance company. Dr. Ratner does not have a contract with your
insurance company, so she is out of network.

Not all services are not covered by benefits in all contracts. Some
insurance companies arbitrarily select certain services they will not
cover.

We must emphasize that, as your medical care provider, our relationship
is with you, not your insurance company. All charges are your
responsibility from the date that services are rendered. We accept cash,
check, MasterCard, Visa, Discover, and American Express.

We will provide a copy of your superbill to submit to your insurance
company for reimbursement.

As of 2024 Dr. Ratner has “opted out” of Medicare. Medicare
guidelines forbid our office from submitting any claims to Medicare
directly. Patients will be asked to sign a “private
contract” indicating that they are responsible for full payment at
the time of the visit and that the visit fee will not be reimbursed by
Medicare.

Reimbursement for any tests or tissue specimens sent out of the office
by Dr. Ratner will be covered at the discretion of Medicare or your
private insurance company. We will make our best effort to refer to
in-network providers whenever possible.
